Family Guidelines at the Ashtabula Nursing and Rehabilitaion Center
Visitation

We have open visitation at the Ashtabula County Nursing & Rehab Center. We ask that visitors be mindful of the time of day and the Residents' personal schedule and/or tolerance. In order to safeguard our Residents we do ask that all visitors leave by 8 pm at which time the front doors will be locked. If your loved one is critical, we will accommodate your hours.

Smoking

No smoking is permitted inside the facility. Smoking is only allowed in outside areas. Residents are allowed to smoke inside only in our designated smoking room and only during designated smoking times. No smoking material is allowed to be in the Residents room. All smoking items will be locked up in the smoke room when the Resident is not smoking.

Personal Belongings

A Resident is encouraged to bring his/her personal effects as space allows. These sentimental "homey" items help to make his/her room their home. They are pleasant reminders of the past that has not been set aside. We do ask that articles or furnishings be cleared through the Director of Housekeeping, Social Services Director, the Director of Nursing, or the Administrator as we must adhere to the state regulation and safety codes.

Leave of Absence

We are most pleased when families can take a Resident off the premises for leaves of short or long durations. All leaves must be scheduled in accordance with the Resident's medical ability. If a leave is not medically advisable, the supervisory nurse will advise the family and enter such into the Residents Medical records.

Advance notice is appreciated so the supervisory nurse can arrange for dispensing of medications.

Holiday leaves are encouraged. We will make every effort to accommodate your schedule. We ask, if you have a clothing preference, that this be made known several days before the holiday so staff can plan their schedule in a timely manner. We may ask that you let us know ahead of the holiday, when you plan to pick up your loved one and the time of return, so we can plan our work flow.

Room Assignments

Room assignments are made in accordance with census availability. We make every effort to see to the compatability of each Resident allowing time for adjustments. Every effort is made to maintain each Resident in their own comfortable environment. Due to changing health status, emergencies or census change, placement in an alternative room may be necessary. Every reasonable effort will be made to contact the responsible party as far ahead as possible, prior to the change. If unable to reach the responsible party prior to a change, then every reasonable effort will be made after the move, informing the party of the Residents new location.

Physicians Choice

We have qualified physicians who come to our facility. Dr James Chillcott and Dr A.S. Lee are our Medical Directors. Dr. Chillcott is located out of the North Kingsville area as well as the Ashtabula Clinic. Dr A. S. Lee is located on the west side of Ashtabula with his office in the "El Grande" steak house building. Dr Gary Huston also sees Residents here. He is out of Conneaut, Ohio and has an office located behind the Conneaut Dairy Queen. Before admission, Residents and/or responsible party members will have their choice of which physician they would like to cover while here.

Pharmacy

Pharmed Pharmacy out of West Lake, Ohio covers our pharmaceutical needs and supplies. A family is welcome to use a pharmacy of their choice. The pharmacy must be able to promptly fill medications when needed, and deliver the medications to the nursing home within 24 hours of receiving telephoned and faxed prescriptions.

Clothing

The Residents wear their own clothing which we can mark upon admission for them or the family can label the clothing themselves. Due to limited space we ask that a reasonable amount of seasonal clothing be brought. Clothing out of season should be stored at home if possible.

Please keep in mind that the type and style of clothing may vary from what the Resident previously wore. Physical and medical needs and limitations may dictate these changes. Any questions or comments are welcomed. Please direct these to the Social Service representative on the appropriate wings.
